Requests for certificates should be submitted in writing, although telephone requests from a Catholic parish or agency are acceptable. No information from sacramental registers will be provided by telephone or other electronic means except to another Catholic parish or agency. Certificates can be mailed to Catholic parish or agency directly or maybe picked up in our Faith Formation offices with photo ID.
